⚠️ This tracker is a structured working aid for a DMAIC (Define-Measure-Analyze-Improve-Control) Six Sigma / Lean Six Sigma improvement project, broadly aligned to ISO 13053-1:2011 and ISO 13053-2:2011 (Quantitative methods in process improvement — Six Sigma). It is not a certified project-management record — verify root causes and results with objective data, hold a real tollgate review with the project sponsor/champion at the end of each phase, and route the finished report through your own management-system document control before treating a project as formally closed.
