AMAADOR · PRACTICAL GUIDANCE
From a risk assessment to an action people can verify
A fictional warehouse example showing how to turn a draft assessment into an owned, reviewable prevention action — without confusing a calculated score with approval.
Prepared for AMAADOR SELFSERVICE · Publisher: Youssef AMAADOR · Published 18 October 2026. Fictional examples, not a site-specific approval.
Start with the task, not a risk score
Atlas Sample is a fictional warehouse with a pedestrian route crossing a loading area. The assessor records the crossing location, shift, traffic pattern, people exposed and existing barriers before choosing a scoring method. “Vehicle movement” alone is too broad: it does not identify who meets what hazard, where or when. The example is a way to structure evidence, not a traffic-management design or permission to operate.
Prepare evidence that another reviewer can understand
A useful starting record contains a clear task boundary, a dated observation, the people consulted, a description of existing controls and a named competent reviewer. Keep evidence references rather than unnecessary personal details. Record uncertainty: a missing observation must not be replaced by an invented frequency or an optimistic score. If the local method uses likelihood and severity, document its definitions and apply the same definitions throughout the assessment.
Build a verifiable action
Weak action: “Be careful at the crossing.” Better action record: “Site manager to review separation at crossing C with operators and logistics, record the selected control and confirm its effectiveness before closure.” Add an owner, target date, interim arrangements and the evidence needed to close the action. Which control is appropriate must be decided on site by competent people. PPE or a poster does not automatically resolve a layout problem.
Use the tool, then challenge its output
Open the assessment tool, enter the organization and task, review each hazard and save the local draft. Treat the generated register as a working record. Ask whether it includes non-routine work, contractors, changes between shifts and people who may need additional consideration. Do not copy this fictional situation into an operational assessment without observing your own workplace.
Approve and follow up
The reviewer should check the evidence, the assumptions and the practicality of the actions, not only the appearance of the file. Record who approved the assessment, when it will be reviewed and which changes trigger an earlier review. Close an action only when implementation and effectiveness evidence are available. Reopen it if the control is not working. A colourful document is useful only if it supports a real decision.
What this example does not establish
There is no universal acceptable score supplied here. The tool is not an external certification, a legal compliance decision or an authorization to perform hazardous work. Use the applicable local requirements and your site method. The official HSE risk-assessment guidance below provides a starting framework; jurisdiction-specific duties may differ.
