Run an anonymous safety-climate questionnaire in the NOSACQ-50 style across seven safety-culture dimensions, then aggregate the results: per-dimension scoring (1–4), a 7-axis radar benchmarked against good practice (>3.0), a department comparison table, a narrative reading of the weakest dimensions and an ISO 45001 action plan — exported to Word, Excel and CSV.
This survey is anonymous. Do not enter your name or any identifying detail. Choose your team and role, say how strongly you agree with each statement, then export your response as a small file to give to the person running the survey — or they can transcribe it.
Use this if you tallied paper surveys. Enter the mean score (1.0–4.0) per dimension. Leave blank to use only imported responses; manual values, where present, override the imported average for that dimension.
⚠️ This is a screening safety-climate survey to structure a safety-culture review — it is not a validated psychometric instrument and it is not the official NOSACQ-50. Items here are written in the NOSACQ-50 style for indicative use; for benchmarking or research use the free official NOSACQ-50 from NRCWE. Keep every response anonymous; never combine answers with HR data in a way that could identify a person. Scores show where to look and act, not who to blame. Have a competent safety professional and worker representatives interpret the results and design the action plan.
Inspired by NOSACQ-50, the Nordic Safety Climate Questionnaire (National Research Centre for the Working Environment, NRCWE — free for research and practical use); this tool is an independent, indicative adaptation and is not the validated questionnaire. Supports ISO 45001:2018 §5.1 (leadership and worker safety commitment) and §5.4 (consultation and participation of workers). Keep responses anonymous.
